Format for an Appeal

If you wish to write a letter to the CSAC outlining the basis or your appeal, the following is a suggested format for compiling the information needed.

You must notify the CSAC of any change in your or your representative’s address while your appeal is pending.
Your daytime contact number/email:
You must notify the CSAC of any changes to your telephone or email address while your appeal is pending.
Department in which you work:
State the department in which you now work or used to work
Name and address of person who took the action or made the decision you are appealing.
State the name and designation of the person or persons who made the decision and attach a copy of the decision if it is in writing.
State the decision that you are appealing.
Specify the action that you are appealing.For example, state whether you have been dismissed; whether you were suspended for 3 days; whether there has been a reduction in your salary, etc. If you are appealing the score for your performance appraisal, state the score you received and specify whether you are appealing the overall score or score of a specific section of the performance plan. Also state whether the decision is the result of an internal appeal.
Body of the Appeal
State why you are appealing the decision or action.In plain, concise language, set out the reasons why you believe the decision was unfair; provide details, including sufficient facts on the ruling or action.Attach any supporting documentation.You must also identify the specific section within the Law or Regulations that you consider the Chief Officer violated.
State the remedy that you are seeking:
Tell the CSAC what remedy you would like to receive.

I declare that the statements made in this appeal and any attachments are true, complete and correct to the best of my knowledge and belief.
